Per New York State Law, you must submit MMR (measles, mumps, and rubella) immunization record and meningitis response form to the QC Health Service Center before attending your orientation. You will not be allowed to register for fall courses if you fail to supply this, and you may be asked to reschedule to a different orientation date.
You may fax your completed form (please note: it must be signed AND stamped by your physician) to (718) 997-2765. Please visit the Health Service Center website at http://qcpages.qc.cuny.edu/Health_Services/ for more information about immunization requirements, free and low-cost clinics with vaccines available, and to download the Health Service immunization form. You may also call at (718) 997-2760. Just in case, it is also a good idea to bring copies of your immunization records with you to orientation. |
If you took any AP exams and/or completed any College Now coursework during high school, be sure to have the official documents (i.e., scores and transcripts) submitted to the QC Office of Undergraduate Admissions (Jefferson Hall, 1st floor). It is also a good idea to bring copies of your AP scores and College Now courses with you to orientation, so that you can discuss with an academic advisor how they will apply to your QC degree. |
