authorize the Department of FNES to add me to the below listed course
at a time when a space becomes available up through February
5, 2004 for the Spring Semester. |

I am obligated
to remove my name from the list if at a later date I choose not to
be added to the course. |
| 3) |
Should the Department
add me to the course and I no longer am interested in taking the course,
I am obligated to drop the course myself. |
| 4) |
It is my responsibility
to use the WEB or telephone system for verifying my placement into
the course. |
| 5) |
If I have not
been placed into the course by the week prior to the beginning
of the semester, I must return, and restate my interest in
registering for this course by placing my initials in the box provided
on the original over-tally list. |
| 6) |
Failure to initial
this sheet prior to the first day of classes will, at the discretion
of the Department, forfeit my place on the list. |
| 7) |
I must verify
that I am signing the list for the correct course
and that the section does not create conflicts with any other courses. |
| 8) |
Listing of the
names in multiple lists will invalidate all entries (only one entry
per student). |
| 9) |
Beginning with
regular registration, Priority will be as follows (by the # of credits):
1) graduating seniors who have not fulfilled the
physical education requirement; 2) physical education majors;
3) graduating seniors who have completed the physical
education requirement; 4) students by class standing. |
| 10) |
I must contact
the Bursar's office immediately to make payment arrangements." |
